All winning players of any weekly raffles are responsible for any and all taxes. Any winnings of $1,500 or more will require the winner to fill out IRS Form W-9 (Name, Address, and SSN). Once the W-9 is filled out, received by the Hollywood Vol. Fire Department Treasurer, and verified the winner will receive the winning prize. A IRS Form W-2G will be issued by the Hollywood Vol. Fire Department at the end of the calendar year.

Any winnings of $5,000.00 or more will require the HVFD to withhold federal taxes. If the winner is unable or unwilling to provide the W-9 information within 30 days, the Jackpot or prize will become the property of Hollywood Vol. Fire Department.
